2006 ASA WEST GIRL'S 10-UNDER "B" FAST PITCH NATIONAL CHAMPIONSHIP FINALS

REQUIRED PRE-TOURNAMENT PAPERWORK

All managers or their business managers need to provide the following information before their team is officially in the tournament.

The original tournament entry form from your state/metro commissioner. This entry form MUST be signed by either your state/metro commissioner or the Tournament Director of the tournament where your team qualified in order to be an official entry form. Original copy is requested to be in ASA Metro St. Louis office not later than 26 July 2006.

A check or money order made out to Metro St. Louis ASA in the amount of $350.00 should be mailed as soon as possible. Do not send via regular mail after 22 July 2006 or express mail after 24 July 2006. Failure to provide check by 26 July 2006 will result in your team not being listed in the tournament program.

    The original ASA roster, front copy of four part form, must be completely filled out with all the required information and signed in accordance with ASA requirements by . Electronic reproduction or a carbon copy is NOT acceptable. This roster must be RED LINED following the last player listed on the roster. Original copy is requested to be in the Metro St. Louis ASA office not later than 26 July 2006.

   Each team is requested to submit the following team information as some as possible to the Per Tournament Planning Coordinator or FAX 314-429-1705: name of team manager; name of team; home town of team; email address of team manager; home and business phone number of team manager; cell phone of team manager; name of team coaches and name, playing position, and number for each player on team. This information will be used to contact the team manager if necessary and provide information for the tournament program.

    If the above information is not provided a team risks the possibility of not being entered in the tournament and there will not be any communication information available to the tournament committee to contact with your team.

    The submission of player date of birth verification information is optional as part of the pre-tournament paper work. However, if this information is not provided as part of the pre-tournament paper work the team check in process will take much longer. If this information is provided early it must be in the Metro St. Louis ASA office not later than 26 July 2004 to avoid a time consuming team check-in. Teams that are checking in late must provide player date of birth verification information as part of their pre-tournament paper work.

    Teams that are using "Virtual Tournament Director" player identification cards to meet the requirement for player date of birth verification are required to contact Per Tournament Planning Coordinator for additional instructions.
