What are the requirements for date-of-birth verification
documentation?
Ans. ASA Code Article 501 ( A ) (6 ) addresses Player Date-of-Birth
Verification and states the following:
"06. Junior Olympic Birth Verification. For each member of a Junior Olympic team, one of the following forms of birth verification or proof of age must be attached to the team's roster/affidavit form: Birth certificate, baptismal certificate, hospital certificate, driver's license, a photo ID card issued by a local ASA Commissioner and/or their designated deputy or a valid passport. In the case of a Junior Olympic player whose name has changed from that which appears on his/her birth certificate, baptismal certificate, hospital certificate, driver's license or passport by reason of adoption or a court approved name change, a copy of the court order or decree approving such adoption or change of name shall also be attached to the roster/affidavit form. Photocopies of any of the above are acceptable."
What will be the Tournament Committee's method of verifying date
of birth?
Ans. The Tournament Committee will be adhering to the intent of ASA
Code Article 501 (A) (07) and (06).
Teams that are using "Virtual Tournament Director" player
identification cards to meet the requirement for player date of birth
verification are required to contact Per
Tournament Planning Coordinator for additional instructions.
When
will player date-of-birth verification take place?
Ans. Verification of player's date of birth will take place as part of
the team check-in and player photo identification and player sign-in processes.

Does date-of-birth verification information have to be sent as
part of the pre-tournament paper work?
Ans. The submission of player date-of-birth verification information
is optional as part of the pre-tournament paper work. However, if this
information is NOT provided as part of the pre-tournament paper work the team
check-in process will take much longer. If this information is provided
early as part of the pre-tournament paper work it must be in the Metro St. Louis
ASA office not later than Wednesday, 26 July 2006.
